I’m not sure myself, but it’s always used in the context of war. That is, we have to fight wars and attack other people so that we can remain free. I don’t buy it.

It means that we have had to fight for our freedom for the last 230 years. it hasn’t come free. Many lives have been lost fighting for the freedoms we have in the US, today.
